What causes crooked or uneven lips?
Almost no one has a completely symmetrical face. Small differences between the left and right sides of the lips are normal, and for many people they have simply always been there.
A number of things can contribute to this. Genetics and natural facial structure play the biggest role, often linked to how the jaw or underlying bone developed. Muscle tone and habitual expressions can also shape the lips slightly differently on each side over time. In some cases, a previous injury, dental work, or old scarring can leave one side looking subtly different to the other.
Ageing adds another layer. As the lips gradually lose volume, an asymmetry that was barely noticeable in your twenties can become more visible later in life. In most cases, a degree of lip asymmetry is simply a normal feature of your face rather than a sign that something is wrong.
Can lip filler correct lip asymmetry?
Yes, in many cases. According to the NHS, dermal fillers add volume beneath the surface of the skin, most commonly using a gel made from hyaluronic acid, with results typically visible almost immediately and lasting between six and eighteen months depending on the product and the individual. For lip asymmetry, this generally means adding a small, targeted amount of filler to the flatter or thinner side to visually even out the shape, rather than treating both sides identically.
A postmarket clinical study looking specifically at hyaluronic acid lip filler found that patients treated for lip asymmetry reported an improved sense of balance in their lip shape following treatment.
It is worth being realistic about what filler can and cannot do. It can improve how balanced your lips look, but it does not change the underlying bone or muscle structure that may be contributing to the asymmetry in the first place. If the difference is largely down to jaw or dental alignment, filler can soften the visual impact, but it is unlikely to be a permanent fix.

When should you see a doctor?
In most cases, lip asymmetry is simply a natural feature and not a medical concern. That said, it is worth speaking to a doctor rather than a cosmetic practitioner if any of the following apply.
- The asymmetry appeared suddenly, rather than being something you have noticed for as long as you can remember
- It is accompanied by drooping, weakness, or numbness anywhere else on the same side of your face
- You have noticed new swelling, pain, or a change in sensation around your lips or mouth
- The change developed alongside a headache, slurred speech, or any other unusual symptom
Sudden changes like these are uncommon, but they should always be checked promptly rather than assumed to be purely cosmetic.
